The clinical review panel was asked to look at the early outline proposal for the provision of sustainable health and social care in West Hertfordshire, in the context of the Five Year Forward View and respond to the question: “in the context of the case for change and developing national recommendations on care models, do the proposed models of care for the future in West Hertfordshire constitute reasonable proposals to deliver high quality care based on known evidence and good practice?”
