Independent, clinical and strategic advice for health systems in the south east

Leicester, Leicestershire and Rutland (LLR) STP Community Health Services

West Leicestershire Clinical Commissioning Group (CCG) commissioned the East Midlands Clinical Senate to review the implementation of their Community Health Services model, which was launched on 1st December 2019. The review focussed on the reconfiguration of existing service provision across Leicester, Leicestershire and Rutland (LLR) and appraised the Home First offer, Integrated Neighbourhood Team model and Locality Decision Unit clinical access points. A second clinical review is due to take place later in 2020, which will focus on the Community Bed Based Model of Care.

LLR is an accelerator site for the National Ageing Well Aging Programme. The model shared with the senate support LLR to deliver on the urgent community crisis response offer and achieve the 2 hour and 2 day response times.
