Oxfordshire Transformation proposals – Phase 1

The proposal involved re-organisation of services across two acute sites and an intention to reduce DTOCs by implementing ‘discharge to assess’. This involved multi agencies and included both community and residential providers. The initial Senate report was completed in November 2016 and an appendix was issued in January 2017 following submission of additional evidence.
