The Bradford Provider Alliance led a redesign of the integrated diabetes service to deliver an end to end diabetes pathway and patient journey from prevention through to acute intervention. The Senate was asked to review the proposal and advise whether it would deliver the outcomes set by commissioners, identifying any clinical concerns and highlighting potential improvements to the model.
